Click here to download printable form

This informal complaint procedure is designed to resolve issues of disability discrimination and/ or obstacles to access of services, programs and activities of the College of Southern Idaho (CSI). CSI uses a collaborative approach providing timely and effective intervention to resolve such issues. CSI works to keep the complainant and other key persons central to the resolution with a focus on achieving a positive change.

Action To Be Taken:

Students, employees or members of the community with such a complaint, complete an ADA/504 Complaint Form and submit it to the compliance office.

The compliance officer will contact the complainant to discuss the application of the complaint to the ADA and/or Section 504 of the Rehabilitation Act.

If these laws do appear to apply to the complaint, the officer will investigate the complaint and return to the complainant to discuss effective intervention, agreement, corrective action or other resolution.

If a resolution can be agreed upon by those parties involved, a written agreement will be prepared and signed by those with roles and responsibilities regarding its implementation.

If resolution can not be established, the complainant may file an appeal to the office of the President of the College of Southern Idaho, who will respond to the complainant in writing.

If the complainant is not satisfied with the response given by the president, other legal remedies may be pursued (Office for Civil Rights/US Department of Education; Idaho Department of Labor and Industries; Office of Federal Contract Compliance, etc.)

All records of the complaint process, including the complaint form and all findings, are confidential and remain the property of the College. During the investigation, information will be shared with and collected from only those persons directly involved with the complaint.
