Application Process

The following are the steps a student needs to take to apply for Veteran Affairs (VA) educational benefits:

  1. Chapters 30, 33, 1606 and 1607: Complete the Application for VA Education Benefits (VA Form 22-1990).  If you have received VA educational benefits while attending another school you should complete VA Form 22-1995 instead.  To apply online click here.  You may pick up the paper application from the CSI Veterans Certifying Official, Carol Carlile, who is located at the CSI  Admissions and Records Office inside Taylor Administration Building, or download it at http://www.va.gov/vaforms.  In addition to submitting your application to the VA, you will need to supply a hard copy to Carol Carlile.

    Chapter 31: Student who are receiving VA educational benefits under Chapter 31 need to contact the VA Vocational Rehabilitation Officer, Matt Osinski, at (208)334-1063 to apply.

    Chapter 35: Complete the Application for Survivors’ and Dependents’ Educational   Assistance (VA Form 22-5490).  If you have received VA educational benefits while attending another school you should complete VA Form 22-5495 instead.  To apply online click here.  You may pick up the paper application from the CSI Veterans Certifying Official, Carol Carlile, who is located at the CSI Admissions and Records Office inside Taylor Administration Building, or download it at http://www.va.gov/vaforms. In addition to submitting your application to the VA, you will need to supply a hard copy to Carol Carlile.
  2. Meet with the advisor for the program you are interested in. If you are not sure who your advisor is, contact the Advising Center located inside the Taylor Administration Building. Have the advisor assist you in choosing and registering for classes.
  3. Complete the CSI Veteran Certification Form.  You may download the certification form or you may pick it up at the CSI Admissions and Records Office from Carol Carlile.  The CSI Veteran Certification Form should be completed for each semester you attend CSI and receive VA benefits.
  4. Meet with Carol Carlile, CSI’s Veterans Certifying Official, to turn in the CSI Veteran Certification Form and confirm you have completed the certification process.  You will need to complete the certification process each semester you are enrolled in school.
