CSI is upgrading student systems from Wednesday, March 20 at 9 p.m. until Monday, April 1, 2024 at 8 a.m. Some systems will be offline during this transition. Students can still access Canvas, email, and more. See how these updates might impact you.

Admission Steps

The Surgical Technology Program is three (3) semesters in length, fall, spring and summer. Students will begin the program of study in August and graduate the following July.

Twelve (12) to sixteen (16) seats are offered annually based on student portfolio, student interviews and local clinical availability. Portfolios are due May 15.

Surgical Technology applicants will be contacted for interview by email only, using the contact information provided on the cover sheet of the portfolio.

  1. Build Portfolio

    Before you submit your portfolio, please do the following:

    1. Make application to CSI
    2. Transfer your previous college work (transcripts) to CSI. Contact CSI Registrar office with any questions.
      1. Do not send to program director.
    3. Contact Financial Aid for help.

    Your portfolio should include all of the following:

  2. Submit Portfolio

    • Students will submit their complete portfolios in paper by mailing them to the program director or hand delivering to HSHS Office Staff.
    • Students can mail their complete portfolio to the address listed below and the postage stamp will serve as the date and time stamp. (No email, no fax)
    • Students will be selected for interview based on the points earned in the portfolio, and the class is selected following the interviews.
      • The class will be selected following the interviews based on the following criteria:
        • Points from your portfolio (using the published score sheet)
        • Your interview ranking (by the committee)
        • Coursework completion ratio (how many credits attempted vs how many credits achieved)

    Mail to: Surgical Tech Program Director, College of Southern Idaho, PO Box 1238, Twin Falls ID 83303 or hand in portfolios to HSHS Office Staff.

    Students who are not admitted will need to reapply the following year. Submitted portfolio or documents will not be kept.

Post-Acceptance Steps

Steps to be taken after accepted into the Surgical Technology Program:
  • Completed H&P and Immunizations. 
    • Take both documents to a health care provider. Students may need to provide a prior immunization record to their provider.
    • The program does not need students' records, only the document filled out completely by the physician
  • Current CPR (BLS) card that does not expire before your surgical technology graduation date (July 30th).
  • Background check results
    • Background Check Policy - Review policy before moving on to the process step.
    • Background Check Process - American Databank/Complio (www.csidahocompliance.com) American Databank does the background check and Complio is the immunization tracker. This is the only background check that will be accepted. Students who have questions regarding this process can contact HSHS Office Staff at 208-732-6702.
