Admission Steps

Applications are due June 1. Only twelve (12) seats are offered annually. 

Those submitted on or prior to June 1 each year will be given preference to those submitted after the due date. The program will accept applications through August, if needed, until the class is full.

  1. Build Portfolio

    A portfolio is made up of all of the following:

    • A portfolio cover page must have each of the following:
      • Applicant's name
      • Applicant's address
      • Applicant's phone number
      • Applicant's e-mail address
    • Student Admissions Scoring Sheet
    • Surgical Technology application
    • Current Resume
    • A notarized letter from a current employer stating the hours student currently or will be working (students who are working the night shift - after 10pm - will not be admitted to the program)
    • Unofficial Transcripts
      • Include unofficial High School, GED, or HSE records and College transcripts
      • Official transcripts must be on file with the Office of the Registrar
    • Completed H&P and Immunizations. Take both documents to a health care provider. Students may need to provide a prior immunization record to their provider. The program does not need students' records, only the document filled out completely by the physician
    • Current CPR card that does not expire within the next 2 years
    • C.N.A. certificate, EMT certificate, or Medical Assistant Certificate (if applicable-it is NOT mandatory)
    • Background check results
      • Background Check Policy - Review policy before moving on to the process step.
      • Background Check Process - American Databank/Complio (www.csidahocompliance.com) American Databank does the background check and Complio is the immunization tracker. This is the only background check that will be accepted. Students who have questions regarding this process can contact Gina at 208-732-6702.
        • Students should give themselves plenty of time to get the results back prior to submitting their portfolio
        • This information is required by the Fair Credit Reporting Act
  2. Submit Portfolio

    Students will submit their portfolios on a flash drive. Please check to make sure all the documents on the portfolio are accessible. If the Program Director cannot open the files, students will not get credit for including them.

    Due to the Corona virus in 2020: Submission of applicant's portfolios will be accepted ONLY by mail. 

    Students must mail their flash drive to the address listed below and the postage stamp will serve as the date and time stamp. (No email, no fax, etc)

    Applications are still due on June 1, 2020.

    Students will be selected for interview based on the points earned in the portfolio, and the class is selected following the interviews.
