Admission Steps

Starting with the class admitted for FALL 2022, the Surgical Technology Program will be three (3) semesters. Students will begin the program of study in August and graduate the following July.

 Applications are due May 1st each year. Only fourteen (14) seats are offered annually. 

Applicant interviews for the Surgical Technology program will be held on the Tuesday and Wednesday following graduation, an invitation will be sent to those selected!
  1. Build Portfolio

    Before you submit your portoflio, please do the following:

    1. Make application to CSI
    2. Transfer your previous college work (transcripts) to CSI. Contact CSI Registrar office with any questions.
      1. Do not send to program director.
    3. Contact Financial Aid for help.

    Your portfolio should include all of the following:

    Note: Documents should be submitted single-sided. Submit your documents in this order: 

    • A portfolio cover page must have each of the following:
      • Applicant's name
      • Applicant's CSI student ID number
      • Applicant's address
      • Applicant's phone number
      • Applicant's e-mail address
      • A current photograph on the page
    • Student Admissions Scoring Sheet
    • Surgical Technology application
    • Unofficial Transcripts
      • Submit ONLY CSI unofficial transcripts
    • Current Resume with a picture
    • Submit a signed copy of the Student Work Policy
    • Submit a signed copy of the Travel for Clinical Rotations Agreement
    • Completed H&P and Immunizations. 
      • Take both documents to a health care provider. Students may need to provide a prior immunization record to their provider.
      • The program does not need students' records, only the document filled out completely by the physician
    • Current CPR (BLS) card that does not expire before your surgical technology graduation date (July 30th) (you will not get points for an expired card).
    • Copy of your CSI BTC (Basic Technical Certificate) for Central Sterile Processing
    • Background check results
      • Background Check Policy - Review policy before moving on to the process step.
      • Background Check Process - American Databank/Complio ( American Databank does the background check and Complio is the immunization tracker. This is the only background check that will be accepted. Students who have questions regarding this process can contact HSHS Office Staff at 208-732-6702.
        • Students should give themselves plenty of time to get the results back prior to submitting their portfolio. If the background check results are not ready by the application submission date, submit payment receipt in portfolio. If granted an interview, bring the results with you.
        • This information is required by the Fair Credit Reporting Act
  2. Submit Portfolio

    • Students will submit their portfolios in paper by mailing them to the program director.
    • PLEASE DO NOT submit your portfolio in page protectors, binders or folders! (Paper clips are preferred)
    • Students must mail their portfolio to the address listed below and the postage stamp will serve as the date and time stamp. (No email, no fax, etc)
    • Students will be selected for interview based on the points earned in the portfolio, and the class is selected following the interviews.
      • The class will be selected following the interviews based on the following criteria:
        • Points from your portfolio (using the published score sheet)
        • Your interview ranking (by the committee)
        • Coursework completion ratio (how many credits attempted vs how many credits achieved)

    Mail to: Surgical Tech Program Director, College of Southern Idaho, PO Box 1238, Twin Falls ID 83303 or hand in portfolios to HSHS Office Staff.

    Students who are not admitted will need to reapply the following year. Submitted portfolio or documents will not be kept.
