Admission Steps

Thirty (30) students are accepted each fall and spring semester, dependent on clinical placement. Given the competitive nature of the program, acceptance is determined on the highest points accrued, see PN Admission Score Sheet*. There is not a wait list; students not accepted will need to reapply.

  • The score sheet will only be used to rank applicants. There is no minimum score needed to apply. Applicants will only receive points for documentation they provide.

Deadline

Application materials are due or postmarked by December 9 or June 1 of each year.

Prerequisites

  • Applicants must be able to meet the mental and physical eligibility requirements.
  • Upon submission of the Practical Nursing application, you must have successfully completed the following with a "C" or better:
    • MATH 143 College Algebra OR MATH 153 Statistical Reasoning
    • BIOL 105 Human Structure & Function OR BIOL 227 and BIOL 228 Human Anatomy & Physiology I and II
    • ALLH 101 Medical Terminology
    • ENGL 101 English Composition
    • PSYC 101 General Psychology
  • Must have a current CNA certification
    • All required Math and Science courses must be taken within 5 years of the application deadline to count toward application scoresheet, program acceptance, and program completion.

 

  1. Follow the steps below to apply to the CSI Practical Nursing program.

    Apply to the College of Southern Idaho.

    *Acceptance to College does not guarantee placement to the program.

    If a transfer student, request to have previous official college/university transcripts sent to the College of Southern Idaho Registrar or request through the National Student Clearinghouse, to be submitted to CSI before the LPN program application deadline.  This process could take several weeks.

     

  2. Submit the following, postmarked by the application deadline, in order to receive points on the score sheet:

    • Application to the Practical Nursing program.
    • Current copy of CNA license (front and back)
    • All unofficial transcripts (CSI included)
    • Copies of any of the following, if applicable:
      • Copy of health-related field credential (i.e. RT, EMT, Paramedic, ST, MA, etc.)
      • Documentation of health-related work
  3. Submit the application and copies personally to the Health Sciences and Human Services main office or mail them to:

    A packet of information regarding the following requirements prior to starting the program will be sent to you upon acceptance:
    • AHA Healthcare Provider CPR card
    • Current immunizations
      • Including COVID-19 (Healthcare partners require students to be vaccinated for COVID-19)
    • Current physical
    • Background check
    • Drug screen
    • Health Insurance
